Mini-Grant Proposal click here for WORD Document Always follow forms provided and supply information for EVERY portion of the application. Most applications will request the following information. If there is no application form, write a proposal using the following format. It is best to write a cover letter stating which grant you are applying for and the amount of money you need for your project. Applicant(s) Date Address Email IntroductionSummarize who, what, when, where, and why. GoalOne or two sentence description of planned results. Needs Statement What will be accomplished? Emphasize student achievement that will be gained through the project. What problem will be solved? Who will be served? Be specific. How many? Project ObjectivesList measurable* outcomes to be attained. ActivitiesDescribe activities (methods) that will be use to meet objectives EvaluationState how project will be assessed based on the objectives above. PersonnelList personnel involved and responsibilities of each. TimelineState when each activity and assessment will take place. Project BudgetInclude cost of materials, transportation, equipment needed to conduct activities. Sharing/disbursement of resultsDescribe a forum that will be appropriate for sharing project outcomes with colleagues, parents, etc. HINTS Keep it short – write succinctly Submit proposal on or before deadline Write a thank you letter when the grant is awarded |
